Chicago Fire will introduce a character from Sylvie's past in the form of Hope (played by Eloise Mumford) in the show's upcoming sixth season.
In a report from TV Guide, exec producer Derek Haas has offered new details about the newcomer and how she'll figure into Hope and Severide's storylines in the new bunch of episodes.

Hope might be a bit of a misnomer.
She's a disrupter, she's an instigator. She's from Brett's small town [in] Indiana who comes to the big city and gets a nice eyeful of what Brett's life is like. [She] likes what Brett's done with herself since she left and [thinks] she should get a piece of what Brett has but might not be willing to put in the same amount of work. So Hope has her eyes on various characters including Severide (Taylor Kinney) and she might not always have the best intentions.

The Season 6 premiere picks a second after the last shot of Season 5. You can expect not everyone coming back for this season.
